MINOR PARTICIPANT WAIVER AND ASSUMPTION OF RISK

Prior to game play, all participants must sign a waiver and assumption of risk form. Minors, must have a parent sign on their behalf

The undersigned below a minor child, and his or her parent or guardian below, knowingly and voluntarily, make and grant this Waiver and Assumption of Risk in favor of Chubbuck Lions Club (the “Lions”), and its agents and assigns as partial consideration, in addition to monies paid to the Lions, for the opportunity to use the facilities, equipment, materials and or other assets of the Lions; and/or to engage in the activities, events, sports, festivities and/or gatherings sponsored by the Lions;

I/we do hereby waive and release any all claims, whether in contract or of personal injury, bodily injury, property damage, damages, losses and/or death that may arise from the aforementioned use or receipt. I/we understand and recognize that there are certain risks, dangers and perils connected with such use, and/or receipt, which I/we hereby acknowledge have been fully explained to me/us and which I/we fully understand, in which I/we nevertheless, accept, assume and undertake after inquiry and investigation of extent, duration, and completeness wholly satisfactory and acceptable to me/us.

I/we further agree to use my/our best judgment in undertaking these activities, use and/or receipt and to faithfully adhere to all safety instructions and recommendations, whether oral or written. I/we hereby certify that I/we am a competent adult assuming these risks of my own free will, being under no compulsion or duress, or that I/we am the Guardian or parent of the undersigned minor child participating in such activities or receipt with my consent.

This waiver and assumption of risk is effective from August 2, 2024 to August 3, 2024 inclusive, and may not be revoked, altered, amended, presented or voided without the express prior written consent of the Lions.
